Z01.818 vs Z01.810: Pre-Op Clearance Codes Compared

How to choose among Z01.810, Z01.811, Z01.812, and Z01.818 for a preoperative examination, and what the Tabular List says each code covers.

Every pre-op clearance visit starts with a code from subcategory Z01.81-, Encounter for preprocedural examinations. The harder question is which of the four codes to pick. Z01.810 and Z01.818 are the two most often confused: one names a cardiovascular examination, and the other is the home for any preprocedural examination that the other three codes do not describe. This guide compares them using the FY 2027 Tabular List and Official Guidelines.

The four codes in Z01.81-

  • Z01.810, Encounter for preprocedural cardiovascular examination.
  • Z01.811, Encounter for preprocedural respiratory examination.
  • Z01.812, Encounter for preprocedural laboratory examination. The Tabular List includes blood and urine tests prior to treatment or procedure.
  • Z01.818, Encounter for other preprocedural examination. The Tabular List includes preprocedural examination NOS and examinations prior to antineoplastic chemotherapy.

When Z01.810 is the better choice

Choose Z01.810 when the record shows the purpose of the encounter was a cardiovascular examination before a procedure. A typical example is a surgeon asking for a cardiac risk evaluation, documented as a cardiovascular or cardiac clearance. ICD-10-CM asks you to code to the highest level of specificity the documentation supports, so a documented cardiovascular exam should not be reported with the catch-all Z01.818.

When Z01.818 is the better choice

Choose Z01.818 when the documentation describes a preprocedural examination that is not specifically cardiovascular, respiratory, or laboratory. Because preprocedural examination NOS is an inclusion term at Z01.818, a general pre-op history and physical documented only as a preoperative examination lands here. So does an examination before antineoplastic chemotherapy.

Z01.818 is not a default for every clearance. If the provider documents that the evaluation was cardiovascular or respiratory, the more specific code describes the encounter better.

Z01.811 and Z01.812

Z01.811 fits a documented respiratory or pulmonary evaluation before a procedure. Z01.812 fits an encounter for preprocedural laboratory work, such as blood and urine tests drawn before surgery.

Rules that apply to every Z01.81- code

  • Z01 codes represent the reason for the encounter. The Tabular List note at category Z01 says a separate procedure code is required for any examinations or procedures performed.
  • Z01 codes may only be reported as the first-listed diagnosis, except when records for multiple same-day encounters are combined.
  • Pre-operative and pre-procedural laboratory Z codes are for encounters where the patient is being cleared for a procedure and no treatment is given.
  • The reason for the surgery is an additional diagnosis, and any findings related to the pre-op evaluation are coded as well.
  • Z01.81 includes radiological and imaging examinations done as part of a preprocedural examination. Testing ordered to evaluate a sign or symptom is excluded from category Z01 and is coded to the sign or symptom.

A quick way to decide

  • Documentation names a cardiac or cardiovascular evaluation: Z01.810.
  • Documentation names a respiratory or pulmonary evaluation: Z01.811.
  • The encounter is for pre-procedure blood or urine tests: Z01.812.
  • The documentation says preoperative examination without naming a system, or the exam precedes chemotherapy: Z01.818.

Questions coders ask

Is Z01.818 acceptable for a cardiac clearance?
When the provider documents a cardiovascular examination before the procedure, Z01.810 is the more specific code. Z01.818 is for a preprocedural examination that the other Z01.81- codes do not describe, including one documented without further detail.
What does Z01.818 include?
The FY 2027 Tabular List includes encounter for preprocedural examination NOS and encounter for examinations prior to antineoplastic chemotherapy at Z01.818.
Can a Z01.81- code be listed second?
No. Category Z01 is on the guideline list of Z codes that may only be reported as the first-listed diagnosis, apart from combined same-day encounter records.
Where does the reason for surgery go?
After the Z01.81- code. The condition that is the reason for the surgery is assigned as an additional diagnosis, followed by any findings related to the evaluation.
