Reason for visit
Follow-up for heart-failure medication management and chronic-condition review.
Interval history
Weight and breathing are at the usual baseline. The patient reports no new orthopnea or leg swelling. Blood-pressure readings remain above the individualized goal, prompting review of the heart-failure regimen.
Findings
Examination shows no acute volume overload or respiratory distress. Recent thyroid testing is reviewed and remains within the treatment target; hypothyroidism is clinically stable.
Assessment and plan
Hypertensive heart disease with chronic diastolic heart failure is assessed without acute decompensation. Adjust the heart-failure regimen and continue home weight and blood-pressure monitoring. Hypothyroidism, cause unspecified, remains stable; continue current management.
